Entry Format Guidance

Use these verbs where possible:

• Added: new capability, file, config, prompt, command, or workflow.
• Changed: modified existing behavior.
• Fixed: corrected a bug or bad output.
• Removed: deleted obsolete behavior or files.
• Documented: clarified usage, schema, or operational rules.
• Verified: records tests/checks that passed.
